In preparation for sign on you can print and complete the following forms:

If your child has never played for a football club before in Australia or New Zealand, you are required to register online at MySideline.  Click here to access.

In addition, you will need to bring your child's original birth certificate or passport with you on sign on to finalise registration.

If your child is transferring from another rugby league club in Australia or New Zealand, you will need to complete the Player Registration and Medical Advice form above and bring to sign on. We will need to request a clearance to our club from your previous club. We can start this process for you in advance if you email your child's name and date of birth to us advising us to request a clearance to the club. Our email address is admin@forestlakejrlfc.com.au.

Sign on is 2nd February from 10am to 2pm. If you cannot attend sign on day you can sign up on training nights from 5pm.
